  • Scholarship Application - Please download and email this form to Dawn Ferrier if you are interested in applying for a College Scholarship for this season.

    Please download a copy of the Manager's Application form, and fill out the form completely. Then fax the form to the League Registrar.
  • Parent's Code of Conduct
    Please print out a copy of the Parent's Code of Conduct Form, read, date, print name and sign the form. Then fax or email the form to the League Registrar.
  • Manager's Code of Ethics
    Please print out a copy of the Manager's Code of EthicForm, read, date, print name and sign the form. Then fax or email the form to the League Registrar.
  • Sponsorship Form PDF format 
    Please print out a copy of the Sponshorship form and follow the link to active.com to sponsor a team(s). For additional information, please contact the Sporsorship Director.
  • Tournament Letter of Intent Upper Division   Lower Division  
    Please print out a copy of the Letter of Intent and return it to your manager prior to their tournament selection meeting.
  • Team Volunteer Data Please print out a copy of the Volunteer Data, fill out and return to the League Registrar.
  • Game Incident Report Please print out a copy of the Game Incident Report and return it to the canteen for review by the rules committee.
